Outdoor School Teachers
SPECIAL NEEDS INFORMATION FORM - OUTDOOR SCHOOL
submit this form ASAP Please describe any special needs your students may have and the extra support required for their success. Submit this form even if you do not anticipate any special needs. CLASS LIST, CABIN GROUPS, STUDY GROUPS
FOR OUTDOOR SCHOOL submit this form three weeks before you arrive on site Please submit an alphabetical list of boys and an alphabetical list of girls. Arrange cabin groups of 2-4 students, distributed as evenly as possible across 6 girls cabins and 6 boys cabins (with a minimum cabin size of 2). If your class is attending the Angelos, Arrah Wanna or Howard sites in the spring, please also assemble four academically successful mixed-gender study groups. If you are attending a fall session, or the Canby Grove or Namanu sites, skip the study group section. Also submit a completed Health History form and Student Behavior Form for each participating student. Extra copies of these forms can be found on the Forms for Parents page. Translated versions of forms and other information in Spanish, Russian, and Vietnamese can be found on the Translated Parent Info page.
MEDICATION LOG - OUTDOOR SCHOOL
bring this form with you when you arrive on site Please fill out this log as you collect students' medications before departing your school. Hand the form along with well-labeled medications to the nurse (or the nurse's medication check-in representative) when you arrive at the Outdoor School site. |
4th Grade Overnight Teachers

Please describe any special needs your students may have and the extra support required for their success. Submit this form even if you do not anticipate any special needs. Special Needs Form for 4th Grade Overnight (Word)
CLASS LIST, CABIN GROUPS, STUDY GROUPS
FOR 4th GRADE OVERNIGHT submit these forms two weeks before you arrive on site Please submit an alphabetical list of boys and an alphabetical list of girls. Arrange cabin groups of 3-5 students and one parent chaperone of the same gender. Cabin groups may share a building with a cabin group from another class. Assemble four academically successful mixed-gender study groups with parent chaperones. If you have 3 classes from your school coming at the same time to the Kiwanis site, or 4 classes coming from your school at the same time to the Kuratli site, please contact the Site Supervisor for an alternate form.
Also submit a completed Health History form and Student Behavior Form for each participating student. Extra copies of these forms can be found on the Forms for Parents page.
